Limousine Prom Contract |
Teen passengers, parents, and limousine service
agrees to the following: |
| 1) No alcohol, other intoxicants, or cigarettes are allowed in the limousine |
| 2) All packages are inspected by the chauffeur and go into the trunk of the vehicle |
| 3) The privacy screen remains open at all times |
| 4) The chauffeur will phone the parents or return the passengers to the original pick-up location if alcohol or drugs are detected. |
| 5) Each parent of a teen passenger must provide a phone or cell-phone contact number in case of violation or emergency |
| 6) Only teens registered on the contract are allowed transportation in the limousine |
| 7) Signing the contract indicates that it is accepted by parents and passengers listed and that the representative from the limousine company has the permission and right to inspect all packages and call parents accordingly |
| Each passenger should supply the following: |
| 1) Passenger's Name |
| 2) Passenger's Signature |
| 3) Parent/Guardian Name |
| 4) Parent/Guardian Signature |
| 5) Phone Number for Parent/Guardian |
| The date of the prom and the signature of the limousine service representative should also be included on this portion of the contract |
